Duramax broken crank

I replaced the crankshaft seal behind the harmonic balancer, replaced the bolt, torqued it to 74ftlbs and then added 90 degrees. Put it all together and fire it up. Within a few seconds the harmonic balancer bolt has backed itself off. It is on a 2015 Sierra 3500hd with the 6.6L if that changes anything.

Continuing to drive the truck wil allow the balancer to slip on the crank and it can play hell on the nose of the crank and the inside of the balancer. Broken cranks seem to be a very rare problem for Duramax's.
